Weber Human Services (WHS), located in Ogden, Utah serves a growing urban population residing in Weber and Morgan Counties.

We are an organization dedicated to helping those in need rebuild their lives and reach their full potential. WHS was organized by a cooperative agreement between the Weber and Morgan county governments to serve as the local authority for the provision of their substance abuse, mental health and senior service programs.

Weber Human Services is the only organization in the State of Utah providing these services under a single administration. Through dozens of human services programs, WHS helps thousands of its neighbors by supporting and empowering our most vulnerable groups including at-risk youth, the elderly, children, youth and adults suffering from mental illnesses, and those recovering from addictions.

Weber Human Services, MACS Plan, is seeking a compassionate and dedicated Social Worker to join our team. We specialize in providing community-based care solutions for adults and seniors who prefer to receive care in environments of their choice rather than skilled nursing facilities.

As a Social Worker with Weber Human Services, MACS Plan, you will play a crucial role in assessing and supporting the health, safety, and social needs of our clients. Your responsibilities will include:

• Conducting monthly visits to Assisted Living Facilities, Apartments, Skilled Nursing Facilities, and client homes to assess their overall well-being.
• Collaborating closely with clients, families, medical professionals, and Assisted Living staff to ensure our clients receive optimal care.
• Completing thorough and accurate documentation of client assessments and care plans.
• Working with Medicaid to coordinate services and ensure compliance with regulations.
• Demonstrating flexibility and adaptability in responding to the diverse needs and circumstances of our clients.

Qualifications:

• Bachelor's or Master's degree in Social Work or a related field.
• Current licensure or eligibility for licensure as a Social Worker in the state of Utah.
• Experience in geriatric social work or healthcare preferred but not required.
• Strong communication skills and ability to collaborate effectively with interdisciplinary teams.
• Compassionate and empathetic approach to working with clients and families.
